| Use this form to place an order for tickets. After you complete the form, click the Submit button at the bottom of the form to send your order to us. When we receive your order, we will send you an email to confirm ticket availability and provide instructions for payment. | | 
Note: An asterisk before an item indicates that the information is required. We cannot process your order without this information. 1. From the Ticket Handling selection list, choose whether to pick up your tickets at the door or have them mailed to you. (Click the arrows to view the pop up list.) 2. Type in the Number of Tickets of each type (Adult or Student) that you wish to purchase. Do not leave empty: type zero (0) if none. 3. From the Performance Date selection list, choose the date on which you wish to attend. (Click the arrows to view the pop up list.) 4. Type in your name, mailing address, telephone number, and email address in the text entry boxes provided. You can use the tab key to move from one text entry box to the next. 5. Use the optional Comments text entry box for any comments or special requests you wish to make. 6. If you want to clear the information and start over, click the Reset button. 7. Click the Submit button to send the information. When you do, your order will be automatically emailed to us. | |
